TEMP Case Manager - Cypress Station
Position Summary
The HGCY Case Management Specialist provides intensive case management, follow-up services, and ongoing support to youth participants enrolled in the Houston Gulf Coast Youth (HGCY) program. This position works closely with participants engaged in education, training, employment, and workforce readiness activities to support successful outcomes and long-term career development. Work is performed under the supervision of the Professional Career Advisor and/or Education and Training Specialist as part of the implementation and monitoring of individualized education, employment, and career plans previously developed for youth participants.
Key Responsibilities
· Provides comprehensive case management and follow-up services to HGCY youth participants enrolled in education, training, employment, or workforce development activities
· Conducts regular contact sessions with participants to monitor progress, identify barriers, and determine if additional supportive or wrap-around services are needed to support retention and successful outcomes
· Connects youth participants to Workforce Solutions staff, community resources, educational supports, and partner organizations as appropriate
· Maintains detailed, accurate, and timely documentation within the appropriate management information systems regarding participant progress, services provided, outcomes, and follow-up activities
· Assists participants with goal setting, career exploration, education planning, employment readiness, and barrier reduction strategies
· Supports participants in maintaining engagement in training, employment, or educational activities through consistent coaching, encouragement, and follow-up
· Coordinates with employers, training providers, schools, and community partners to support participant success and employment opportunities
· Ensures compliance with HGCY, WIOA Youth, and Workforce Solutions program requirements, documentation standards, and performance measures
· Participates in outreach activities, workshops, hiring events, and recruitment initiatives to support youth engagement and enrollment efforts
Qualifications
· Bachelor’s degree in business administration, education, social work, psychology, human services, or a closely related field
· An equivalent combination of post-secondary education and/or job-related experience may substitute for the minimum qualifications on a year-for-year basis
· Experience working with youth populations, workforce development, education programs, case management, or social service programs preferred
· Strong communication skills and community resource knowledge, including partner organizations for referrals and supportive services
· Analytical and problem-solving skills
· Proficiency in Microsoft Word and Excel
· Ability to work with diverse populations and individuals facing barriers to education or employment
· Demonstrated ability to work effectively in a dynamic team environment with strong interpersonal, communication, and customer service skills
· Excellent organizational and time management skills with a high level of professionalism
· Knowledge of WIOA Youth and workforce development program requirements preferred
· Ability to thrive in fast-paced environments
· Bilingual or multilingual preferred
